If paying by credit card, select the tests and number of tests and checkout through the website. Continue through the checkout process and select the credit card option for payment. Your tests will be sent to the "ship to" address in your FTS registration.
If your agency would like to pay via purchase order, the initial steps are the same. When selecting payment, select the "Print and Call" option, write the PO number in the "Special Instructions" area of the receipt and instead of calling the number on the receipt, please FAX the receipt to 517-579-4847.
Please note that test prices are subject to change until ordered.
Prices include shipping within the U.S. and Canada. Orders placed outside the US and Canada will be billed a surcharge of $40.00 USD per test to cover additional shipping expenses.
